Payment/Deposit Instructions:

 

Payment may be made at 250 Clark in person, or online by etransfer.  Payment of the first week of enrollment is due now to hold your place for the remainder of the session. Every other week's payment is due by the THURSDAY OF THE WEEK PRIOR. Failure to pay will result in cancellation of existing bookings until the owing balance is cleared.

You will only have to complete the registration section once. 
Both weekly (available now) and daily (available in June) online links will continue to be available for the summer.
Please do not send payment until you receive confirmation of availability by text or email. 
All etransfers require the last name of your child, plus the number of children included in the payment, and the week(s)  you are paying. Example: Marshall02 Week2 (two kids with the same last name, sent in one payment) 

Weekly Rate: $120 First child, $100 Second and thereafter 

Daily: $30

Cancellation/Illness/No-Show Information:

Our ability to run this program for the community depends heavily on balancing enrollment and staffing in a cost effective manner. To that end, no shows, or cancellations (due to illness or scheduling changes) will not be refunded. When possible, with notice, transfer will be allowed, within our ability to identify space and meet supervision ratios. No shows or cancellations who have booked spots, fail to pay and communicate with the program will not be permitted to attend until the balance owing is cleared. Previously booked spaces will be cancelled. 

It is expected that parents will diligently screen for symptoms of all illness and only return children to the program once they are symptom free or have been improving for 24 hours.
 Any cancellations required due to staff illness will be circulated by email at the earliest possible time.

POWASSAN RECREATION PARTICIPATION AGREEMENT
By signing this document you will waive certain legal rights, PLEASE READ CAREFULLY.
IN CONSIDERATION of allowing my minor child(ren)/ward to participate in the programs, activities and events of the Municipality of Powassan Recreation Department/ I ASSURE TO YOU THAT:
1. I am the parent/guardian of the above named participant(s) having full legal responsibility for decisions regarding the above named participant(s).
2. I believe that my child(ren)/ward is physically, emotionally and mentally able to participate in the programs, activities and events of Powassan Recreation Department
3. I hereby acknowledge that I am aware of the risks and hazards associated with or related to physical sport.
4. Furthermore, I am aware that my child(ren)/ward may:
a. Sustain injuries in organized sport that can be severe, cause spinal cord injuries and even be fatal;
b. Experience anxiety while challenging himself/herself during the activities, events and programs;
c. Come into close contact with other participants, including the possibility of accidental and unexpected contact;
d. Risk of injury is reduced if he/she follows all rules established for participation; and
e. Risk of injury increases as he/she become fatigued.
I UNDERSTAND AND AGREE, on behalf of myself, my heirs, assigns, personal representatives and next of kin that my signing of this document constitutes:
5. I am registering my child(ren)/ward willingly and my child(ren)/ward is participating voluntarily in these activities, events and programs.
6. I agree that there are risks in sport as described above and my child(ren)/ward will be exposed to these risks and hazards.
7. I agree to accept all these risks and hazards and be responsible for any injury or other loss, which my minor child(ren)/ward might receive while participating in these events, activities and programs.
8. If something happens to my child(ren)/ward, I release the Organizers of responsibility for any claims, demands, actions and costs, which might arise out of my child/ward’s participation. I understand “Organizers” to mean: The Municipality of Powassan, volunteers, Staff and program participants.
Accident Insurance
Executing this agreement will not preclude you from accident insurance coverage, subject to the terms and conditions of The Municipality of Powassan’s insurance policy.
I ACKNOWLEDGE MAKING THIS AGREEMENT
By signing and dating below, you agree that you are the parent or legal guardian of the player(s) being registered and to be bound by this Legal Agreement even if you have not read the agreement. Digital submissions without signature will be considered acknowledged and signed.
ZERO TOLERANCE POLICY
A zero tolerance policy exists within the Recreation Programming to protect participants and staff against bullying, name calling, teasing, foul language and violence. Every effort is made to work with participants and allow them the opportunity to change their behaviour when they are misbehaving. However, a zero tolerance policy exists to protect all those involved, and clearly demonstrate the repercussions of committing excessive verbal or physical abuse towards other participants and/or staff. This behaviour would include, but not be limited to: hitting, punching, kicking, or other forms of fighting, verbal threats of violence, and excessive name calling and swearing/foul language. Furthermore, any person caught stealing or damaging another persons property will also be subject to the zero tolerance policy. The previously mentioned behaviours will not be tolerated and will result in the immediate expulsion for the remainder of the day of camp with no refund provided. At the discretion of the staff and management, a child will be able to return the following day/week, however, a second incident will result in permanent exclusion from any programming.

I also understand that it is my responsibility to provide transportation for my child/children from the premises should an incident be reported to me.
